Membership Application Form
Please download the Membership Application Pack (follow the link button below) and complete the enclosed forms using one of the following options:
1. To complete electronically
Download and enter the information into the form electronically, then use the SUBMIT button to send.
2. To complete manually
Download and print the forms, fill them in, scan and email to membership@hae.org.uk.
Alternatively you can send us the completed forms by uploading them below.
All applications are subject to a 2-week cooling off period. During this time the Membership Team will be in contact to create your full profile and put together your Members Information Pack.
